In this section, we described the procedure for taking action in the event of an insurance event, as well as a list of documents that must be submitted to the insurance company in order to receive a decision on the payment of compensation

WHAT SHOULD I DO?

  1. Contact us immediately at the phone number indicated on your insured person's card, in case of any deterioration of health.
  2. Notify our coordinating doctor:
    • Full Name.
    • Individual card number of the Insured Person.
    • The reason for the application: complaints, the need to obtain a doctor's consultation, the need to purchase medicines, etc.
    • Contact phone number.
    • Location.
  3. Follow the coordinator's recommendations for receiving medical services.

WHAT DOCUMENTS ARE NEEDED?

Documents required for payment of insurance compensation from the Insured:

  • If the medical service was organized by the doctor-coordinator of "Euroins Ukraine", you do not need to provide any document, we will take care of all expenses for the provision of assistance.
  • If, upon prior agreement with the coordinating physician, you paid for medical services yourself, to receive compensation you must provide:
    • application for payment.
    • passport and ITN.
  • • financial documents confirming the fact of payment for services: checks, receipts, etc.
  • medical documents: prescriptions, doctor's advisory opinions, acts of performed work, research results, etc.
